Frequently Asked Questions
Is there an age limit to purchase this membership? Anyone who is within the age range of 14-80 is eligible to apply. Age 14-16: they have to be accompanied by a parent/ legal guardian with a signed parent/guardian consent form and show proof of identity of their parent/ guardian during medical treatment/ consultation; Age 17-18: they have to present a signed parent/guardian consent form and show proof of identity of their parent/ guardian while the presence of their parent/ legal guardian during medical treatment/ consultation is highly recommended but not required. The parent or guardian shall also provide personal information for registration purpose. Customers under the age of 18 need to use the parent or guardian’s membership account for activation.

How can I activate BowtieGo Dietitian Consultation Plan? You will receive an SMS or email from SUPREME which will provide the activation code. You may visit https://www.supreme.vip/home/vas/dietitian/activate-en.html and click the "Activate Now" button. Enter the activation code and personal information in order to activate BowtieGo Dietitian.
How can I make appointment for consultation and checkup? You can login to app.bowtie.com.hk and then select "BowtieGo Dietitian Consultation Plan" under Health Service of My Protection. You will be redirected to Bowtie & JP Health website for making an appointment.
In what form does the dietitian consultation take place? Customers can choose to conduct the consultation online or in person. For online consultation, customers will meet the assigned Dietitian via WhatsApp or any other designated software application(s). As for consultation in-person, customers will meet the assigned Dietitian at Bowtie & JP Health. The first consultation is recommended to be in-person for the best result. No medication or supplement nor prescribed diet will be prescribed.
What is the format and service time of the unlimited access to dietitian advice from Dietitian over WhatsApp? Applicable after the first dietitian consultation), via WhatsApp and in text format. The service time is from 9:30am to 1:30pm and from 2:30pm to 6:30pm on weekdays, excluding public holidays. Membership
Is Dietitian consultation suitable for me? Dietitian will work with the customers towards their health goals through advising on what and how to eat well and maintain healthy lifestyle, providing nutritional dietary guidance services, formulating personalized nutritional menus according to individual physical needs and dietary preferences, and providing practical dietary guidance. Is there any expiration to the membership? You may enquiry SUPREME about your membership. The Service will continue to be provided at $69 per month upon expiry of the 24-month contract period unless the customer terminates the Service or otherwise specified. The Customer will be entitled to: (a) one Dietitian Consultation for each 12 month period and the Dietitian Consultation may be used at any time during that 12 month period; and (b) unlimited access to dietitian advice from Dietitians over WhatsApp. |
